Specimen Request's Information/Protocols

Review all information to be aware of any changes to SAB procedures and to ensure a problem free experience!!!! By submitting a request, you acknowledge that you are aware of these rules.

Specimen Lab Studies (On-Site)

This page details commonly asked questions about the form and sections. Please carefully complete the entire form. Incomplete forms cannot be processed and will be returned for correcting. The SAB may contact you for additional information to determine if your request meets the SAB policies for appropriate donor use prior to approval. All date fields must be month/day/year.

1. Date of Request - SAB study laboratories are usually fully booked, and resources allocated several weeks ahead. Short notice requests will be considered but may be availability limited.

2. Study Details - The SAB has a large clinical training lab typically configured for up to 6 tables, a small training laboratory configured for up to 3 tables, and an embalming laboratory for training in embalming technique configured for up to 3 tables. The clinical training laboratories are equipped with wall mounted monitors for displaying study materials to participants, however you will need to bring your own computer to connect to the display system. Specify the number of tables in the laboratory space preferred.

List the number of donors that you wish to use within your study. Disinfected donors have undergone disinfection processes. Embalmed donors have been embalmed with a phenol/formaldehyde solution. Non-disinfected donor has undergone no procedures for reducing pathogen risk. Note, all donors are serotyped and screened for hepatitis and HIV, no donor with those diseases or known/suspected presence high risk pathogens are assigned to study programs.

If you have specific donor criteria essential for your study please list those criteria and the SAB will do its best to fulfil the request (e.g. male, female, etc.). Be aware that narrow donor criteria reduces the likelihood that the SAB will have a donor of the necessary criteria at the time of your study. Examples of narrow donor criteria include post-mortem interval less than 30 hours, presence of specific diseases/disorders such as cancer or dementia, etc. You may incur a lab use fee (but not the body use or PPE fees) if narrow criteria are specified and a donor with the appropriate criteria is not available at the time for your study request. SAB staff will advise you if that is a risk for the criteria you specify.

List the total number of participants that will be present at the SAB facility for your study, this includes instructors, support personal, and participants. Indicate if you are requesting the SAB to provide personal protective equipment (PPE) and be billed for the equipment or if your study is providing the PPE. SAB standard PPE includes gloves, face mask with eye shield, gown, hair cover, and shoe covers. Any study provided PPE must comply at a minimum to the SAB provided PPE standards. Any participant or study not meeting those standards will be required to wear SAB provided PPE and the study program billed accordingly. Wearing of scrubs by participants is strongly encouraged.

You must list the procedures that are being performed in your study (e.g. cricothyrotomy, tube thoracostomy, femoral artery exposure, etc.). This allows SAB staff to allocate suitable donors for use in your study based on any donor preexisting conditions or prior study uses. You are not permitted to perform any procedures beyond those listed on your request. If SAB staff identify that additional procedures have been performed you will incur a second body use fee to cover the non-requested procedures performed as this may disrupt predetermined allocation of that donor to other study programs. This may also become grounds for prohibiting you or your institutions use of SAB donors.

If you have special requests, for example non-standard room configuration, equipment needs, support staff need or other special requests, please provide those detail(s) in the ‘Special Requests’ box. SAB staff will contact you to confirm if the special request can be accommodated for your study.

3. Requestor - This section must be completed in full. The requestor name/contact, organization, and administrative contact are used for the SAB to communicate with you about your study request. The invoice detail section is required for the SAB to bill for the study lab. The reference field is any internal reference you have for SAB correspondence on billing. A valid Federal TAX ID number is required.

4. Note - TWO (2) separate invoices will be issued for each study, one for body use issued by Maryland Dept of Health, and one for laboratory use issued by University of Maryland.

Specimen Request Releases (Off-Site)

The SAB may contact you for additional information to determine if your request meets the SAB policies for appropriate donor use prior to approval. All date fields must be month/day/year.

1. Date of request - SAB resources are allocated weeks to months ahead of time. Short notice requests shall be considered but may be availability limited.

2. Study Details - You must enter a donor pick-up and return date.

For embalmed donors this transport date may be up to two weeks prior to the start of your program and up to two weeks after the conclusion of the program. A study program with embalmed donors may run for one academic semester and longer periods of time may be considered by the SAB with suitable justification.

For disinfected (non-embalmed) donors the transport date may be no more than 24 hours prior to the start of your program and 24 hours after the conclusion of the program. If the facility has suitable refrigeration, a multi-day study may be considered by the SAB with suitable justification.

If making a special request for excised organs note that in the special request field. If your staff performs the specimen removal at the SAB there is a lab use fee for use of the space. If you are requesting SAB technical staff to perform the removal a Technician or Specialist hourly fee will apply.

This allows SAB staff to allocate suitable donors for use in your study based on any donor pre-existing conditions or prior study uses. You are not permitted to perform any procedures beyond those listed on your request. If SAB staff identify that additional procedures have been performed, you will incur a second body use fee to cover the non-requested procedures performed as this may disrupt predetermined allocation of that donor to other study programs. This may also become grounds for prohibiting you or your institutions use of SAB donors.

4. Note - Off site requesters are responsible for the costs and coordination of all specimen transportation to/from the SAB in accordance with Maryland regulations.

5. Invoice - Invoice for fees will be provided by the Maryland Department of Health, 201 W Preston St, 5th Floor, Baltimore, MD 21201. W-9 can be requested.
